Parents & Student-Athletes;

Please note this information is ONLY intended for those with students involved in UIL Athletics at Merkel ISD. Students in the Pre-Athletics or PE classes are not required to complete any of these forms.

All new forms are live and available for completion on the Rank One website or the app. Please use your login information and complete the following forms:

OTC Medication - Discuss our steps for dispensing OTC medications to our student-athletes

Drug & Alcohol Deterrent Policy - UIL and MISD required acknowledgment of the policies and procedures within

UIL Athletic Forms/Signature Page - Acknowledgement of UIL and MISD required information sheets

Emergency Contacts and General Health Information - Updating contact information and verifying basic health questions

SB12 Consent - Acknowledgment and making an authorization or non-authorization about an Athletic Trainers ability to provide: injury evaluation; injury rehabilitation; triage; wound care; and other medical procedures for your student-athlete. Required by Texas law.

Medical History/Physical Upload - Prior Parent Square post has more detail. Skip this form if you did a physical at our mass physical event. If you did not complete one with us please upload the front and back of the completed and cleared UIL physical form. I have to approve each physical that is uploaded.
